Dofetilide is a class III antiarrhythmic agent and inhibits the rapid delayed rectifier K⁺current (IKr) with IC₅₀values of 31.5 nM in guinea pig cardiomyocytes. At higher doses it demonstrates pro-arrhythmic activity in cell models. Therefore, dofetilide has been cautiously used in the treatment of atrial fibrillation, the conversion of flutter to sinus rhythm and in the maintenance of sinus rhythm after conversion.
