The research-grade biosimilar is a humanized monoclonal antibody without the drug conjugation. The original antibody-drug conjugate (ADC) (Belantamab mafodotin) consists of Belantamab, an afucosylated, humanized monoclonal antibody that targets TNFRSF17, a cell surface receptor that belongs to the family of tumor necrosis factor. TNFRSF17 is expressed on multiple myeloma cells but not on naïve and memory B-cells. The antibody is conjugated to monomethyl auristatin F (MMAF) via a protease-resistant maleimidocaproyl linker. Upon binding to TNFRSF17, the ADC is internalized by the myeloma cells. Once inside the cell, the drug is separated from the antibody by proteolytic cleavage. The drug then disrupts the microtubule network, which leads to cell cycle arrest and apoptosis. In addition to drug-mediated apoptosis, the monoclonal antibody induces antibody-dependent cell cytotoxicity (ADCC) and antibody-dependent cell phagocytosis (ADCP) via recruitment and activation of immune effector cells. The original ADC is approved by the FDA to treat patients with relapsed or refractory multiple myeloma, especially those that are refractory to anti-CD38 monoclonal antibodies, immunomodulatory drugs, and proteasome inhibitors
